領収証記入の注意事項

　毎年、年末から翌年３月にかけて、医療費控除のための領収書発行の依頼が増えてきます。
この時期に依頼のある領収書は、「一年分をまとめた領収書」、「紛失による再発行」などが多いのではないでしょうか。
今回は領収書記入の注意事項をあげてみます。

１．１年分の治療費をまとめた領収書は、既に渡してある領収書を必ず回収して記入する。
２．再発行する場合は、再発行の旨を記入する。
３．再発行する時は、但し書きに何日の入金分かを記入し、
　　領収証の日付は、発行した日付を記入すること。
４．書損の場合はバツを書き、必ず正控ともに残すこと。
５．カードで領収した場合は、但し書きにカードの種類を記載のこと。
６．ＢＫ振込分の領収証の発行は、但し書きに振込の日付を記入し、
　　領収証の日付は発行した日付を記入する。
７．なお、３０，０００円（税込）以上の領収であっても、
　　印紙税法第５条及び基本通達２５により非課税です。
